Historically speaking, implants are a recent blip on the centuries-long march of dental progress. But few innovations in dentistry can match the impact of implants in its short history on dental function and appearance.

Dental implant therapy has already established itself as a restoration game-changer. But it also continues to improve, thanks to a number of emerging technologies. As a result, implant restorations are far more secure and life-like than ever before.

Here are 3 examples of state-of-the-art technologies that continue to improve this premier dental restoration.

CT/CBCT scanning. Functional and attractive implants depend on precise placement. But various anatomical structures like nerves or sinuses often interfere with placement, so it's important to locate these potential obstructions during the planning phase. To do so, we're increasingly turning to computed tomography (CT). This form of x-ray diagnostics is the assembly of hundreds of images of a jaw location into a three-dimensional model. This gives us a much better view of what lies beneath the gums.

Digital-enhanced planning. Implant success also depends on careful planning. And, it isn't a one-sided affair: The patient's input is just as important as the dentist's expertise. To aid in that process, many dentists are using digital technology to produce a virtual image of a patient's current dental state and what their teeth may look like after dental implants. This type of imaging also allows consideration of a variety of options, including different sized implants and positions, before finalizing the final surgical plan.

Custom surgical guides. To transfer the final plan details to the actual implant procedure, we often create a physical surgical guide placed in the mouth that marks the precise locations for drilling. We can now produce these guides with 3-D printing, a process that uses computer software to produce or "print" a physical object. In this case, the 3-D printer creates a more accurate surgical guide based on the exact contours of a patient's dental arch that's more precise than conventional guides.

Obtaining a dental implant is a highly refined process. And, with the aid of other advances in dental technology, it continues to provide increasing value to patients.

Dental implants have revolutionized restorative dentistry. Not only are they the top choice for individual tooth replacement, implants also improve upon traditional dental work.

Dental bridges are a case in point. A few well-placed implants can support a fixed bridge instead of natural teeth, as with a traditional bridge. Furthermore, a fixed, implant-supported bridge can replace all the teeth on a jaw.

But although convenient, we can't simply install an implant-supported bridge and forget about it. We must also protect it from what might seem at first an unlikely threat—periodontal (gum) disease.

Although the bridge materials themselves are impervious to infection, the natural tissues that underly the implants—the gums and bone—are not. An infection plaguing the gums around an implant can eventually reach the bone, weakening it to the point that it can no longer support the imbedded implants. As the implants fail, so does the bridge.

To guard against this, patients must regularly remove any buildup of plaque, a thin biofilm that feeds disease-causing bacteria, adhering to the implant surfaces in the space between the bridge and the gums. To do this, you'll need to floss—but not in the traditional way. You'll need some form of tool to accomplish the job.

One such tool is a floss threader. Similar to a large needle, the threader has an eye opening at one end through which you insert a section of floss. You then gently pass the threader between the bridge and the gums toward the tongue.

Once through, you release the floss from the threader, and holding each end, you work the floss along the implant surfaces within reach. You then repeat the threading process for other sections until you've flossed around all the implants.

You might also use a water flosser, a device that directs a spray of water between the bridge and gums. The pressure from the spray loosens and flushes away any plaque around the implants.

Whatever the method, it's important to use it every day to reduce the threat of gum disease. You should also see your dentist regularly for further cleanings and checkups. Keeping your implants clean helps ensure gum disease won't ruin your fixed bridge—or your attractive smile.

Ed Helms is best known for his role as the self-absorbed, Ivy League sales rep, Andy Bernard, on television's The Office. But to millions of fans he's also Stu, a member of a bachelor trip to Las Vegas in the 2009 movie The Hangover. In it, Stu and his friends wake up from a wild night on the Strip to find some things missing: the groom-to-be, their memories and, for Stu, a front tooth.

In reality, the missing tooth gag wasn't a Hollywood makeup or CGI (computer-generated imagery) trick—it was Ed Helm's actual missing tooth. According to Helms, the front tooth in question never developed and he had obtained a dental implant to replace it. He had the implant crown removed for the Hangover movie and then replaced after filming.

Helms' dental situation isn't that unusual. Although most of the 170 million-plus teeth missing from Americans' mouths are due to disease or trauma, a few happened because the teeth never formed. While most of these congenitally missing teeth are in the back of the mouth, a few, as in Helms' case, involve front teeth in the “smile zone,” which can profoundly affect appearance.

Fortunately, people missing undeveloped teeth have several good options to restore their smiles and dental function. The kind of tooth missing could help determine which option to use. For example, a bridge supported by the teeth on either side of the gap might work well if the teeth on either side are in need of crowns.

If the missing tooth happens to be one or both of the lateral incisors (on either side of the centermost teeth), it could be possible to move the canine teeth (the pointy ones, also called eye teeth) to fill the gap. This technique, known as canine substitution, may also require further modification—either by softening the canines' pointed tips, crowning them or applying veneers—to help the repositioned teeth look more natural.

The optimal solution, though, is to replace a missing tooth with a dental implant which then has a lifelike crown attached to it, as Ed Helms did to get his winning smile. Implant-supported replacement teeth are closest to natural teeth in terms of both appearance and function. Implants, though, shouldn't be placed until the jaw has fully developed, usually in early adulthood. A younger person may need a temporary restoration like a bonded bridge or a partial denture until they're ready for an implant.

Implants are highly regarded by both dentists and patients for their versatility and durability. But it’s their life-like appearance that “seals the deal” as the restoration of choice — not only mimicking an individual tooth, but emerging from the gum line and blending indistinguishably with other teeth in color and symmetry.

To achieve this result, we must consider a few factors beforehand, particularly the amount of bone available at the intended implant site. An implant requires a certain amount of bone to properly position it for the most natural crown appearance. The bone present around adjacent teeth can also affect your appearance: in the absence of adequate bone the papillae, triangular shaped gum tissue between teeth, may not regenerate properly between the implant and the natural teeth. This can leave a noticeable void, what dentists call “black hole disease.”

Bone loss is a significant problem particularly after tooth loss. It’s quite possible for you to lose a quarter of the bone’s width in the first year after tooth loss. To avoid this, we often use bone grafting techniques immediately after extraction to lessen bone loss; if it’s already occurred we may be able to use similar reconstructive techniques to rebuild and encourage renewed bone growth. In the end, though, if there remains a significant level of bone loss it may be necessary to consider another option for tooth replacement other than implants.

The thickness of your gum tissue, a genetic trait, can also have an impact on the implant’s ultimate appearance. Thicker gum tissues are generally more resilient and easier to work with surgically. Thinner gum tissues are more susceptible to recession and tend to be more translucent, which could cause the underlying metal implant to be visible. Thus, working with thinner gum tissues requires a more delicate approach when trying to achieve a visually appealing result.

All these factors must be balanced, from implantation to final crown placement. But with careful planning and attention to detail throughout the process, many of these issues can be overcome to produce a satisfying result — a new and appealing smile.

For generations, dentures have helped people avoid the dire consequences of total teeth loss. Now, implant technology is making them even better.

Composed of life-like prosthetic teeth fixed within a plastic or resin gum-colored base, dentures are manufactured to fit an individual patient’s mouth for maximum fit, comfort and performance. But dentures also have a critical drawback—they can’t stop bone loss in the jaw.

Bone is constantly regenerating as older cells dissolve and then are replaced by newer cells. In the jawbone, the forces generated when we chew travel through the teeth to the bone and help stimulate this new cell growth. When teeth are missing, though, the bone doesn’t receive this stimulus and may not regenerate at a healthy rate, resulting in gradual bone loss.

Dentures can’t transmit this chewing stimulus to the bone. In fact, the pressure they produce as they rest on top of the gums may actually accelerate bone loss. Over time then, a denture’s once secure and comfortable fit becomes loose.

In the past, most patients with loose dentures have had them relined with new dental material to improve fit, or have new dentures created to conform to the changed contours of the jaws. But implant technology now offers another alternative.

Implants are in essence a tooth root replacement. Dentists surgically implant a titanium metal post directly into the jawbone that naturally attracts bone cells to grow and adhere to it over time (a process called osseointegration). This not only creates a secure and lasting hold, it can also stop or even reverse bone loss.

Most people know implants as single tooth replacements with a porcelain crown attached to the titanium post. But a few strategically placed implants can also support either removable or fixed dentures. Removable dentures (also called overdentures) usually need only 3 or 4 implants on the top jaw and 2 on the bottom jaw for support through built-in connectors in the dentures that attach to the implants. A fixed bridge may require 4-6 implants to which they are permanently attached.

There are pros and cons for each of these options and they’re both more expensive than traditional dentures. In the long run, though, implant-supported dentures could be more beneficial for your bone health and hold their fit longer.
